Envisage Appointments
Booking Deposits & Cancellation Policy
As a courtesy to other clients (who may not be able to get a booking) and our staff, a $50 deposit is required to secure your booking. Deposits can be redeemed on treatment or refunded on the day. Please give us 48 hours’ notice if you do need to change or cancel your booking so that we have the opportunity to rebook your spot.
*We regret that deposits will not be refunded with less than 48 hours’ notice.
Confirmation Policy
In consideration of our waiting list, we confirm all our appointments 48hrs earlier. Envisage Clinic will call or SMS you 48hrs prior to your scheduled appointment. Could you please confirm your appointment within 2 hours of this reminder or your appointment may be allocated to someone on the wait list.
Patient History
All new patients are required to complete a Patient History Form. This form must be filled in correctly with your details and any other previous medical conditions and treatments.
Envisage Confidentiality
Information about a patient’s medical health history is needed to provide accurate medical diagnoses and appropriate treatment. A strict code of Medical Confidentiality is your guarantee that we will not pass any of your information to another party.
Appointments
All consultations and treatments are by appointment only. We aim to keep to time so please arrive at the clinic five minutes prior to your appointment. If you are a new patient it is advised that you arrive 10 minutes prior to fill out a New Patient History Form.
Late Arrivals
At Envisage Clinic we don’t like to leave you waiting. As a courtesy to other patients and our staff, if you arrive late to your scheduled time we may need to reschedule your appointment. Envisage Clinic believes that it is inconsiderate to other patients if they are kept waiting because you are late. We appreciate your understanding.
Front Desk Staff
Our friendly staff members have fantastic knowledge of all the treatments and procedures that we offer. They will be able to advise you on what treatment may be required for your concern; however they do not do any treatments and cannot give you formal advice.
